Service Incident Reports (SIRs)

Purpose

Sites are asked to complete a Service Incident Report (SIR) following any unscheduled service outage that results in that service being continuously unavailable for a period of 24 hours or more. The purpose of the SIR is to document the timeline of activity associated with the service outage including the cause, actions taken and eventual solution. This information is sometimes rather difficult to extract from the numerous emails lists that not everyone may be able to receive or follow. The SIRs are intended to be easy to complete and will help form a historical record from which all sites can learn and improve overall service availability and reliability.
